Make An Appointment
You have the option to either call our office at (920) 327-7056 or set up an account using our patient portal to make a new patient or follow-up appointment.
If you are a new patient, please print and complete the New Patient Intake Form linked below and bring along to your appointment. If you have a telehealth appointment, please email the completed form to clinic@wagnerintegrativehealth.com before your appointment. For minors, please print the pediatric intake form below. Please also bring along any relevant medications, supplements, labs, or other medical documents to be reviewed at your first appointment.
You do not need to complete the New Patient Documents if you are only seeking an aesthetic appointment.
